Speak how you
actually speak.

English-only voice scribes are the past. Real consultations switch languages mid-sentence — Hinglish in a clinic, Tamil-English in Chennai, Spanglish in Miami, Welsh-English in Cardiff. MediSero+ transcribes natively and structures the note in whichever language you (or your clinic) prefers.

  • Mid-sentence code-switch — say "the BP is one thirty over eighty-five, kuch dizziness ho rahi hai?" and it gets it.
  • Note language ≠ patient language — patient speaks Hindi, you write the note in English. Or vice versa.
  • WER per language surfaced — settings show accuracy per language so you can verify.
  • Hybrid pipeline — low-confidence segments auto-rerun on a more expensive model, transparently.

Custom templates

Make it
your structure.

SOAP works for half of medicine. The other half wants something else. Build your template once: pick the section labels you'd actually write, save, done. Clio uses YOUR structure for every visit tagged with that template.

  • Unlimited templates — one per visit type, one per specialty, one per partner.
  • Test on a real recording — paste a transcript, see the new template's output before saving.
  • Share across the clinic — clinic-admin templates available to every doctor; personal templates stay personal.
  • Built-in for 12 specialties — GP, cardio, derm, ortho, paeds, psych, OB-GYN, endo, ENT, ophthalmology, oncology, surgery.
Drafts only

You sign. The AI never does.

Every output is a draft until you tap Sign. Edit anything. Reject anything. The note doesn't enter the patient record without your approval. Always.

Edit any section inline

Click into any heading or paragraph. Edit. Save. Your edits feed back into Clio so it learns your voice (locally, within your clinic).

Reject + redo

Doesn't look right? Reject the draft, re-record, or paste an edited transcript and regenerate. No partial states make it to the chart.

Sign + permanent

Once you sign, the note is in the visit record with your name, timestamp, and an audit trail of edits. Standard clinical immutability applies.

Audit log forever

Every edit, every regeneration, every signing event is logged with timestamp + author. Regulators and your own QA team can trace any note back to source.

How it works

From tap-record to signed note.

Five steps. Most happen automatically. The doctor's hands stay where they should be — on the patient, then on the keyboard for 30 seconds at the end.

1

Record

Tap the mic. Talk naturally. Tap stop. iOS, Android, or web — same flow on every device.

2

Transcribe

Hybrid pipeline: cheap-fast model first, expensive re-pass on low-confidence segments. Diarised by speaker.

3

Structure

Clio applies your selected template (SOAP, Brief, Detailed, Follow-up, custom). Output appears as draft.

4

Review

Edit any section inline. Switch templates. Add Rx draft alongside. Compare against past visits if needed.

5

Sign

One tap. Note enters the visit record with your signature, timestamp, and audit trail. Done.

I used three other scribes before MediSero. The first did SOAP only and missed Hinglish. The second was English-only and 'enterprise'. MediSero is the first that handles how I actually consult — Hinglish, sometimes Hindi-only, follow-ups in Brief, complex visits in Detailed. I save two hours every day.

Questions

Everything else about voice notes

Is the voice note feature just SOAP?

No. MediSero+ ships Standard SOAP, Brief, Detailed, and Follow-up templates out of the box — and every doctor can build their own custom template from Settings → Transcript Templates. The same recording generates different note structures on demand. Switch tabs, see a different output instantly.

How accurate is the transcription on multi-language consults (Hinglish, Hindi-English mix)?

High. Transcription runs on Deepgram with a hybrid pipeline — low-confidence segments are re-processed with a more expensive model automatically. Hinglish, Tamil-English, Spanglish all work natively. We measure word error rate (WER) per language and surface it in clinic settings if you want to inspect.

Can I edit the AI-generated note before signing?

Always. Every AI-drafted note is in draft state until you tap "Sign". Edit any section inline. Your edits help Clio learn your voice and preferences over time, locally within your clinic — never cross-clinic.

What happens to the audio recording?

Your audio is processed for transcription, then auto-deleted from MediSero servers after structuring (default 24 hours). You can configure retention from Settings → Privacy. The transcribed text and the structured note stay in the patient record per normal clinical retention.

Can I make my own custom template?

Yes. Settings → Transcript Templates → New. Define the section labels (e.g. for dermatology: Lesion description, Differential, Plan, Patient education) and Clio uses your structure for any consult tagged with that template. You can have unlimited templates — one per visit type, one per specialty, one per peer.

Does it handle interruptions, background noise, or unclear audio?

It tries hard. The pipeline marks low-confidence segments and surfaces them in the transcript so you can quickly verify or correct them. Background noise (clinic chatter, traffic, fans) is filtered. If a section is unintelligible, Clio leaves a placeholder rather than hallucinating.

Can I record on one device and review on another?

Yes. Record on your phone during the consult, walk to your desk, the structured note is already waiting on your web app. Real-time sync across mobile + tablet + web.

What about specialties — does it know cardio vs derm vs paeds?

Out of the box, MediSero+ has refined templates for the top 12 specialties (GP, cardio, derm, ortho, paeds, psych, OB-GYN, endo, ENT, ophthalmology, oncology, surgery). Each is tuned for the language and structure that specialty uses. Custom templates layer on top.

How does pricing work for voice notes specifically?

Voice Notes are included on every paid plan — Solo (₹1,499/doctor/mo), Clinic (₹3,999–₹5,999/clinic/mo for 3–5 doctors), Enterprise (custom). No per-minute usage caps for normal clinical use; we monitor for abuse.
